  • The primary objective of this single-blind, multi-center, randomized clinical trial is to study the effects of home-delivered, low sodium, nutritionally robust meals and augmented dietary counseling on readmission burden and health-related quality of life in Veterans with incipient or chronic heart failure (HF). The primary outcome is a hierarchical composite of Days Alive and Out of the Hospital (DAOH) and the change in Kansas City Cardiomyopathy Questionnaire Clinical Summary (KCCQ-CS) between hospital discharge and 6 weeks post-discharge. The primary hypothesis is that, within the 6 weeks immediately following hospital discharge, Veterans receiving nutritional support will have a larger number of days alive and out of hospital, and greater improvement in health-related quality of life, compared to Veterans receiving the study-defined standard of care. The study seeks to enroll 1,400 Veterans aged ���60 years who have been hospitalized (���24 hours) for new HF or worsening chronic HF from 35 medical centers.
